Forms
The following forms are commonly used in the chapter:
- Expense form (.doc). Expense forms must be submitted for all chapter-related purchases. If you buy food for the meeting, use this form to submit to the treasurer for reimbursement.
- Membership Records Order Form (.txt) Use this form to order a copy of all official members of the chapter. The record includes all chapter member names, addresses, contact info, and stc numbers. Submit the form to the STC (the membership assistant). If this staff member changes, consult this STC page to determine the person in charge of membership.
- Community Status Report form. This form allows us to report on chapter activities; it must be submitted quarterly. Contact Cindy Currie, chair of the STC Community Affairs Committee, for more information on submission due dates.
