How to Enter

1. Download the official FTCC Entry Form.
2. Complete the form according to the competition rules.
3. Make four copies of the form.
4. Attach one copy of the form to each of the four copies of your entry. Deadline for submission of entries is September 30, 2011.
   

   •	Entries will not be returned.
   •	Each entry must be an original, not a photocopy.
   •	For periodicals, send 4 sets of 3 consecutive issues.
   •	For electronic submissions accessible by URL, enter the link on the form.
   •	For electronic submissions not accessible by URL, send four USB drives, DVDs, or CDs.
   •	If you have any questions, please review the competition rules and, if the rules don’t answer your questions, send an email to STC.Suncoast.Chapter@gmail.com.

5. Want to pay for your entry fee using a credit or debit card?

Single Author $90.00 Author + One Contributor $125.00 Author + Two Contributors $160.00 Author + Three Contributors $195.00 Six Additional Contributors $210.00


6. To add additional contributors to your entry, include another $35 for each contributor.
7. Pack the four copies of your entry for shipment and place the original form with the check on top.
